Watchword
An anonymous report form for staff, and the violent incident log California Labor Code 6401.9 asks for - kept on your own site, with no outside service.
For California employers and nonprofits with a staff site or intranet on WordPress.
Not legal advice. The written plan, training and the standard Cal/OSHA adopts are still yours to do.

Inside Watchword
The form asks for no name and the plugin stores no user, IP address or browser. An optional "how to reach you" field is there only for those who want a reply.
Reports stay private
Only people who can manage the site see them.
Log entries are written from reports with every element 6401.9(d)(1) lists: date, time, location, type of violence, a detailed description, who committed it by type, the circumstances, where, consequences, whether security or police were contacted and what they did, actions to protect employees, and who completed the entry.
The door
an entry goes into the log only when every element is filled, nothing in it looks like a name, email, phone number, Social Security number or street address, and the reviewer has ticked that they read it for identifying details.
Records
entries in the log are not edited; each shows the date it may be deleted, five years on. Uninstalling deletes nothing.
Requests
record an employee's request for the records and see it due 15 calendar days later.
The shareable copy
the log as a CSV, entries only, never report text.

Daylight
A public records request portal inside your own site: tracking numbers, your state's response clock, extensions with reasons, denials with exemptions, and a public log that never names who asked.

Docketline
Meeting agendas, packets and minutes posted as accessible web pages, not only PDFs - and no meeting published until everything on it can be read by a screen reader.

Evenhand
The Title IX notice your website must show - coordinator contact details and the nondiscrimination statement - checked against 34 CFR 106.8 before it is published, with history and reminders.
